On-going Support

Following the person's move to their service and in addition to the daily support they receive from the staff team at their service, each person living at HQL receives on-going support in the following ways:

  • A person focussed menu outlining all areas of need e.g. Adaptation of Environment
  • Medical and Primary Healthcare Needs review
  • Registrations and regular contact with GP, Dentist and other specialists
  • Weekly monitoring of Positive Behaviour Support and regular input from the Behaviour Team
  • Monthly Medication Reviews from HQL's Consultant Psychiatrist
  • Six Monthly Internal Reviews
  • Annual External Review (in addition to an 'as required' basis)

With this combination of pre-planning and on-going support, HQL aims to maximise the potential for maintaining a person with learning disabilities who may present challenging behaviour in a community setting.
